Becoming a Patient
Thank you for choosing 1st Choice Acupuncture!
In order to serve you better, we prepare a check list of things to prepare before your first visit so you can relax in the hassel free envirornment.
Appointments can be made through phone call or e-mail.
Please download and fill out the "Patient Forms". Bring the forms with you for the first appointment or come in a little bit early to fill out the forms
Expect to spend 1 to 1 1/2 hours for your first visit.
Please wear something loose-fitting and comfortable.
For Facial patients, please download both "Patient Forms" and "Facial Forms" and please don't wear make-ups for your treatments.
